A charge entered late is a claim paid late.
Doctor, charge entry is the least discussed step in the cycle and the one that quietly sets your days in A/R. Nothing downstream can be fast if the charge sat in a pile for a week first.

Capture the encounter accurately
Charge entry connects documented care to the billing record. We review the supplied encounter details, enter the applicable information and flag missing or inconsistent items before they move further into the claim workflow.
Receive the encounter
Enter and validate
Resolve exceptions
The lag nobody counts
Days in A/R is usually measured from the date of service, so a week spent waiting for entry is already lost before submission. It never shows up as a billing problem because no claim exists yet.
Charges that never got entered
An encounter with no charge does not generate a denial or an alert. It simply is not there. Without a reconciliation between visits and charges, missed revenue is invisible.
Filing deadlines that pass quietly
Timely filing runs from the date of service, not from entry. A charge found during a month end cleanup can already be past the window with a payer that allows ninety days.
How we handle charge entry
Here is what the work looks like, from the first review to the next action.
A defined task.
A documented action.
A visible next step.
Entered within one business day
Encounters received today are entered tomorrow at the latest, so the claim clock starts while the visit is still recent.
Checked against the documentation
The charge is read against the note before it is posted. A code the note does not support is caught here, which is far cheaper than a recoupment later.
Reconciled against the schedule
Visits are matched to charges daily. An encounter with no charge is queried the same week rather than found at month end, if at all.
Modifiers applied before submission
Modifiers, units and place of service are set correctly the first time instead of being corrected after the payer rejects the claim.
Held only when a question is real
If documentation is genuinely missing we ask one specific question rather than holding a batch silently. A held charge with no owner is the same as a lost one.
Included in charge entry
Agree the scope with your practice, then keep the day-to-day work organized around it.
- Entry within one business day of receipt
- Charge checked against encounter documentation
- Daily visit to charge reconciliation
- Modifier, unit and place of service review
- Missing charge queries raised to your team
- Timely filing deadlines monitored
- Entered directly in your practice management system
- Monthly reporting on lag and missed charges

Small entry errors can travel a long way
Dates, units, provider details and diagnosis linkage matter downstream. A check at entry can surface a mismatch while the source documentation is still easy for the practice to clarify.
Entered when there is time, or entered on a clock
Charge entry is usually the first thing dropped on a busy clinic day, because nothing breaks immediately when it is.
| Situation | Fitted around the day | How we work it |
|---|---|---|
| A busy clinic day | Charges wait for a quiet hour | Entered within one business day regardless |
| An encounter has no charge | Nobody notices it is missing | Surfaced by daily reconciliation |
| Documentation is unclear | Best guess, or held indefinitely | One specific query, with an owner |
| A modifier is needed | Added after the claim is denied | Applied before submission |
| Staff are on leave | Entry stops until they return | The queue does not know they were away |
A few things to know before we start
The right scope starts with a clear conversation.
Ask it directly. You will get an answer from somebody who works accounts, not a sales script.
Monday to Friday, 8am to 5pm Central TimeHow do we get encounters to you?
However you already produce them. If the notes are in your system we work from there. If you send superbills, we take them the way you send them, securely.
Do you code as well as enter?
Charge entry and coding are separate services and some practices want only one. If your providers select their own codes we enter and check them. If you want the coding done too, that is our medical coding service.
What if a note is incomplete?
We raise one specific query naming what is missing. We do not guess a code, and we do not sit on a batch of held charges without telling you.
Can you catch charges we have already missed?
Often, yes, if they are still inside the filing window. That is one of the things the free audit looks for.
Most practices do not buy one service in isolation. These are the ones that change the same numbers.
